Create feature to add marker to database and show on Mapbox map

I have an existing MapBox map displaying a basemap and circles.
I want you to add the following features to it:
A. Geocode and map locations in MySQL database to the map, placing a marker
B. User can add a record to the database and update the map live:
1. Form with captcha that user fills out (see mockup attached)
2. script adds form inputs to a MYSQL database
3. script geocodes location (based on address)
4. script adds a basic marker
If you can do this please tell me in your proposal your approach so I have confidence in your understanding. I need this work done today and tomorrow.
