Custom Android-based Patient Care Report System

We need a custom application built which will capture about 100 fields worth of data and submit it to a server. The application needs to have many different types of fields including text fields, numerical entry fields, drop-down lists, multi-select lists, and various different types of application logic for manipulating these fields depending on various inputs. The application will have multiple screens to collect various inputs. A good example of this type of application is [login to view URL] The application needs to constantly stay in contact with the server to ensure that the active user is still the active editor (and if not, kick them out), and is constantly storing the information in the application in the server database so that if a person's Android tablet's battery dies, someone else can pick up where they left off without losing the information already entered. If someone logs into the system, and selects a patient, the application should pull down all of the previously entered information from the server and populate all of the fields.
