Cross platform mobile app development

Hi I need a cross platform product (Android, IOS) , developed with cross platform tools like titanium or any other popular tool. I am looking for long term relationship as I will have more apps that needs to be developed in future for my customer. This app will work as checklist fill up or form fill up app for any business. Please let me know duration and [login to view URL] you should have UX, Dev and Testing. All 3 kind of resource for this project. I am expecting best user experience with very inviting look and feel. Requirement 1. The checklist with be in xml for json file. It will have some 5-10 header fields and lists of checklist. The checklist will include qns and format of input (multiple values-drop down, radio button, text, numeric, check box etc), url for instructions doc or video to be displayed with the checklist. 2. App should read the input template file and render screen to get input for header and checklist. The input to check list in proper format plus and comment text input, and attach picture or video. 3. User should be able to save offline(probably as json or xml) and online posting to database. 4. Should be easy way to navigate one checklist item at a time or multiple. And the checklist could have sections, and each section can have groups and then each group could have one or more checklist item. 5. Need to have server side code on java. 6. There should bar code scanner as well.
