Magento - Separate create an account page for wholesale customers

We currently have an Magento store which we use for both retail and wholesale. We are looking to separate the registration process (create an account) between these two and having additional fields for wholesale customers when they register an account. On the standard "create an account" at the moment we have the below, which we are happy to leave for retail customers: First Name Last Name Email Address Password Confirm Password Sign up for newsletter reCaptcha For those looking to open a wholesale account up we would need them to contact us by email first and then we would provide them a link to create an account (which would be the page below), the fields would include the below and would need to be saved in the back end as well: Confirm Wholesale terms (link and check box) Wholesale FAQs Link *First Name *Last Name *Email Address Company Name *Street Address 1 Street Address 2 *Town/City *County *Country (choose from dropdown) *Post Code *Telephone Number *Question 1 (with answer box to fill) *Question 2 (with answer box to fill) *Question 3 (with answer box to fill) *Question 4 (with answer box to fill) *Question 5 (with answer box to fill) *Password *Confirm Password reCaptcha Some text here (* required to fill) The above info would then create them an account (with all answers saved in backend) for us to approve and set them a specific wholesale account.
