Design of two WordPress themes and programming of PHP database with backend access

Details: - Website 1: Setup and design of a professional, modern and visually appealing WordPress website with 4-5 content pages based on STARTER (free WP theme). The website should look good and perform well on all devices (responsive theme) and contain the following content pages: home, about us, what we do, team, contact - Website 2: Similar to Website 1 but in addition including an application form in which users can provide personal data (e.g. name, email, etc.) with checkboxes, free text fields, dropdown menus and upload files (e.g. DOC, PDF) to a database. The database should be accessible for the administrator via a password-protected backend and allow for simple filtering and export of content / data. The application form should not simply create an email, but the content should be stored in a separate database. Similar look and feel as [login to view URL] but not via Google Form but with own database operated on GoDaddy web space.
