In curious coincidence, I also have done a CD package featuring Johnny Cash, as has another bidder on this project. (I, though, won't abuse this service by spamming personal contact information.) My bid is based on my fixed rate for CD packages of $50 per surface, a "surface" being a booklet page, CD face, tray card, etc. Since you specify a 4-page booklet, I assume that you'll need a tray card and a CD face, which is a total of 6 surfaces, times $50 = $300. I've sent a PM to you with a PDF file of samples of my work (without contact information).
