css/html forms

around 8 forms (simple form half of them are repeat elements) - with complete form structure guide & css style sample i would like to use, this will also be inside an iframe (lightbox), so no much design. but u must have the sense of structure design.

basically you create simple page, with links/buttons, click on it, iframe lightbox come out.

the iframe have to be auto scroll if longer than the lightbox size. (lightbox size should be less than 70% of screen or you can propose)

need it urgent! and can be contacted by Skype/Facebook(prefer this so u know who am i) at most time!
