Hi,
I have attached 6 documents and my logo. I would like each of these documents altered as shown below (mostly just have my logo placed on the top instead of the current logo). I would like all of these documents sent back to me as an editable word processing file (preferably google document file). If you need any further instructions please email me at DrC@southingtonortho.com.
1)New Patient Information for Children:
a) Replace graphic at top with my logo
b) Change background to all white or something that matches my color theme
c) Remove fields, add fields, and edit text as noted (please call with any questions)
d) At the top where I wrote “New Patient Information for Minors” please change that to read “New Patient Information for Patients Under 18 Years of Age”
2) New Patient Information for Adults
a) Replace graphic at top with my logo
b) Change background to all white or something that matches my color theme
c) Remove fields, add fields, and edit text as noted (please call with any questions)
d) At the top under my logo write “New Patient Information for Patients Over 18 Years of Age
3) HIPAA
a) Change logo at top to my logo
b) Change address at bottom to: Southington Orthodontics, 1095 West Street, Southington, CT 06489
4) Media Consent: Change logo at top to my logo
5) Truth in Lending: Change logo at top to my logo
6) Truth in Lending for Husky: Change logo at top to my logo and change amount as shown.
7) Letterhead:
a) change logo at top to my logo
b) change colors to my logo colors
c) Change address, phone number, website and address info as follows:
Southington Orthodontics, 1095 West Street, Southington, CT 06489; Phone: 860-406-8220; Fax: 860-406-8250; www.SouthingtonOrthodontics.com, Info@SouthingtonOrtho.com;
