** DUE TO THE DIFFICULTIES IN MAINTAINING PDF FORMS AND BEING ABLE TO STORE THEM PROPERLY FOR LATER REFERENCE I WILL NEED THIS TO BE A WORD DOCUMENT...

Looking to create a form that is WORD based. I want to be able to hand my clients this form to fill out their profile of home theater / etc. equipment so I can program an automation system based on the results. Here is what I need to show:

First Section : Client Information
- Client Name
- Client Zip Code
- Client Email
- Client Phone
- Name of Room(s) to control:
- Name of Cable or Satellite Service:
- Name of Remote:
- Name of Base station or Processor:

Second Section: Client Equipment
- Equipment Brand - Equipment Model - Comments
(need 10 lines of above)

Second Section: Client Activities
- Name of Activity -Input on TV/Projector - Input on Receiver - Port Number -Control Type (IR/RF/RS-232/IP) -Listen in Zone2 Y/N?
(need 10 lines of above)

Third Section: Client Comments
(Client can fill out comments in paragraph form up to 100 words.

ADDED AS PER FIRST WAVE OF ENTRIES: PLEASE READ BELOW
- form can be in PDF, WORD, or EXCEL in order of my preference.
- prefer fields to be named internally in case I want to store the information in a third party program once a client fills out.
- client will fill out form and email it back to me. Form must be able to email to clients.
- FIELDS NEED TO BE MADE HIGH AND WIDE, especially the name of the activity. Some fields that require a Y/N or short answer must be made smaller, but all must have increased height.
- FORM MUST BE 2 PAGES MAX
- FORM MUST HAVE CLEAN, INVITING LOOK

Hi i can create a form in PDF where d user can enter the information and send it back to you and in simple steps like double clicking the file u can add them to a database too.(its a type of data storing method from adobe) if you are interested i can develop it most bank even use this method to collect and store data.

thank you but not interested in any interactive forms. The customer receives the form by downloading from a website or by being emailed. He then fills it out, saves it and emails it back to me. Based on the details provided, I create a control system for him. I then save the file to a local directory on a PC (not a mac) and refer to it in the future if needed.

hello sir,
I have actually made a same kind of work which I made it for REpower Energy SE,
I actually cannot upload it to show you as that is a confidential kind of work, but if you can just give me the main details that you use for the form than I can surely make a demo for u of the form as the form that I make will not be a static kind but a dynamic kind of excel sheet, so that it is easy for sharing between you and the customer with whom you need to share the sheet.
I hope to get some basic details so that I can show you a demo for it and you can see a working demo of that sheet and than I can make a real work if you like my demo presented to you.

  I have uploaded a sample but in it I am unable to show you the interactive kind of form as I am only able to upload Jpeg and different kind of picture files only.
  If you like my template even 40% than I hope you can surely get what you expect in your thoughts as I can than work with you by talking with you and give you the best as you want.

Hi,

I can develop an easy to use Excel application for your requirements. User can fill out a form in Excel and the entered data will be stored in Excel database. I think if you have data stored in Excel, it will be useful for your future automation as well. You can also print a PDF version from the Excel data.

  It can be no more than two pages if possible. Great point - absolutely needs to be edited. Customers make mmistakes all the time, or add information or I have to re-word some things they didnt get right.

Request you to please answer my following questions.
Do you want this form to be filled as physical copy? (i.e. handwritten filled by the customers)
Do you want this form in soft copy (PDF Form) in which customer can fill the details offline and email it back to you. Then you can put the complete data automatically by just clicking single option. That means you don't have to type any thing except clicking on export in the Excel format from PDF database file.

Everyone did a great job - however I need fields to be wider and higher. Fields such as "Name of Activity" needs to be longest. Fields that require a Y/N or three letter code (IR/RF/RS-232) could be made to be shorter. I would like to email the client a word, PDF or last choice, Excel file, have him fill it out, and email it back. I am not interested in another piece of software to track, monitor, collect, etc. Ideally the form could be set up with field names so I can collect data via access, but that is not mandatory, just would be nice. Looking for a clean, simplistic look that allows the client to enter in his information clearly. The form can be more than one page, no more than two. I like the idea of Excel and database features, but sometimes excel forms can be a little intimidating to some clients, so if using excel need ultra clean look. Thank you all!
