Good day awesome QR code designers. I would love you to design a stunning QR code for me please.

I am an Orthopaedic Surgeon and I look after people with hand and shoulder problems mostly in Western Australia. My name is Paul Jarrett, Hand and Upper limb Surgeon and my practise name is the "Murdoch Orthopaedic Clinic"

I am making a booklet for my patients and in it I want to be able to direct them to my youtube video on patient wound care. I need a QR code for this and ideally a QR code for my general website also.

The colour scheme for my booklet is mostly dark and light blue (see the colours of my logo please - I have uploaded this).

I don't just want a QR code that you can designs off the online QR generators.. I could do that myself; I need your help to make it really interesting.

My logo could be in the QR code as a small image or larger image of back ground or it could be coming out of the image. Alternatively an image of a shoulder or hand somewhat associated with the QR code would be super.

I have attached some images of some QR codes I thought were good as well as my logos (MOC). I have also attached a QR code I did myself so you can see what I'm not really interested in as I can do that myself. I have put a photo of me in also in case it is of use although suspect it may not be.

The QR codes would be being scanned by smart phones or tablets.

In terms of output I would need a resolution of at least 300 and would like the image files such as an adobe Illustrator or Indesign or Photoshop with output via jpeg or pdf or eps etc.

Hi... I have updated the brief as I think I was not clear enough as to what I am interested in. I've added some QR codes which are awesome and am interested in something a bit more out of the box. I understand the QR codes I have attached as from large multi-national companies who have spent 1000s of dollars on design and I don't need something as complex as this but it give you an idea. I don't want a simple designs straight from an online QR code generator although the QR code itself could come from an online app it needs somewhat enhanced. :o)
