Website form to Google Spreasheet

I am looking for a solution using a Google script or Wordpress plugin to take form data from a website or the email sent by the form and extract that data to a google spreadsheet. The form would also email the gathered information as a backup. If extracting data from the email (in gmail) the data may look like the following in the email: Phone No: 2222222222 Address: 1216 park dr City: roanoke State: Texas Zip: 76262 The Google script would extract the data. The data would be added as a new line to a Google spreadsheet I am all for using a wordpress plug-in on my site to basically embed the google form but it has to remain formatted like the current form found at: [login to view URL] on the right side.
