WordPress user account section

This is an online meditation platform. Website is [login to view URL], and the project needs help building the user account section in preparation for its early May launch date. **Please reply with which plugins & code you will use for each of the 5 sections below, and total hours. Only replies that include details for the 5 sections will be considered for this project.** 1) "Register" page - Create a form with the questions on this page. [login to view URL] + All fields are required and, once submitted, saved into a database. + Question 2 receives a calendar date input. This will be the user's program start date (on the "Account" page; details below). + Answer to question 5 must be yes to submit the form. + Once submitted, user's account is created based on email and password in question 1. User account access must be installed for this website. + User will automatically receive an email confirming their account creation. 2) Once logged in, user is taken to the "Account" page. [login to view URL] Attached is a drawing of the page layout. This page should only be accessed by the logged in user. It will display a list of dates -- with a check box to the left and notes field to the right of each date. There should be 21 total lines from the start date through the end date. + Text above date list says, "Check off days in which you complete a one-hour meditation session." + Checked off dates and notes are captured in the database as separate fields. 3) On day 22 of their program, the "Account" page automatically redirects them to the "Exit Survey" page. This page should only be accessed by the logged in user. Questions 1 & 2 are number inputs from 1-10. Question 3 is a text input. + User should automatically receive an email with "Exit Survey" page link and a request that they complete the survey. 4) Custom report is displayed on the "Report" page after Exit Survey is submitted.
