Develop an IVR in Elastix (Asterisk) to get customer information

The project consist in the next important considerations:
- create an asterisk IVR for after hours (close time)
- The IVR should ask the customer if he wants to get called in the next day.
- If the customers wants to get called, we should get the phone number from the call or ask a new one to get called the next day.
- After we get the phone number, we need to store in a DB (mysql)
- After we store the phone number, we should ask for ID number and get it from DTMF tones, and also store in the DB (mysql)
- Also they need a name, so i need an idea to get the name, maybe a 5 seconds recording for this information o waht other idea.
So at the end we should have: phone number, ID number and the name (recording or other idea.
