I need a Custom Contact Form using the Wordpress plugin Contact Forms 7. This is going to be for a fitness club free daypass request.
Attached is the contact form structure:

Field aspects:
- Nombre (name, just text, required)
- Apellido (last name, just text, required)
- ¿A dónde te gustaría acudir? (select location, just one, it'll display the checkboxes like in [login to view URL] and [login to view URL], max 4 checkboxes selected, required).
- Correo electrónico (valid email, required)
- ¿Qué día te gustaría acudir? (select date, 2015 by default, required)
- ¿Tienes alguna duda o comentario? (for comments, not required field)

We've done dozens of forms using CF7 - including anti-spam measures.
This is pretty straight-forward stuff. We'd:
- Take a complete backup of your existing site
- Install Contact Form 7
- Install Contact Form 7 Honeypot (a hidden anti-spam tool)
- Install Contact Form DB (backs up any emails, for easy access later)
- Configure the new contact form
- Test and deploy
This can be done within 2 working days.
