Need a from for my site (HTML, php, java, Jquery)
5 field with radio box questions
3 field with radio box or drop box ** (one with 15 image product, one with 60 image product, 5 categories with some sub categories)
10 field with standard product with quantities box
5 text or number field ( 1 email, 2 name, 3 your customer name, 4 your message, 5 phone)
** For this field need to have image preview so my customer can easy choose the product, my products is leather style, so need to look very good before you choose it,

One tricky point is that if you choose in one drop down some specific values, then you cant choose all the value in the next field
also if you choose product from the firth drop down field then cant use the second drop down or opposed, and then you proceed to the three drop down field.
