Create a dynamic logic website contact order form (NOT Wordpress)

I have a PHP/HTML/CSS website that needs a dynamic-logic contact/order form added. There are about 6-10 choices to be made by the user, and depending on previous inputs, the fields should either show or not show. At the end, the "custom order" is pieced together, and must go to a Paypal payment button where, depending on the choices made in the order form, a certain amount of payment will be taken. Could be more, could be less, depends on what the user chooses.

This website unfortunately does not run on any CMS platform or Wordpress. So you need to manually code this, or use a 3rd party FREE application to create this. No additional license fees or anything should apply.

The CSS for this website is very basic. You might have to add some CSS to make this look order form look "pretty" and make the form fields etc. graphically stand out and/or integrate with the current look of the website. We are going for a good user experience here to make completing this form as easy and smooth as possible.

After the form has been completed, an "order email" must be sent to the website administrator.

The website already exists in a live online demo version. I can show this to you in a PM if you want to see it before bidding.
