Convert VBA Excel form / database to a basic web application

This database and form was written in VBA Excel and needs to be converted into a web format, so people don't have to email it back and forth or rely on Google Drive / Dropbox synchronization.
Additional functionality would include a user login and depending on your access you could have visibility over all submission or just items belonging to your "club or association".
The webmaster and committee members of BSNSW (very few people) will be able to see and modify services requests, approve revise or reject things.
Once a service request has been assigned to a person, the person need to be notified that there is something waiting for him to action.
if this is done in a MySQL or MS SQL database, it might be a good idea to split the database table into a few small tables. That way the revisions and updates don't have to be all in the comments, we can create activities under each SR.
Users will be in a separate table.
Application also needs to work on mobile devices. so keep that in mind when designing forms.
I have done many of these in my time and it is not a very complex requirement. We will organize hosting for the work.
All bidders, need to provide reference of their work or samples of similar work and also provide time estimates of how long this would take to perform.
Strong English communication skills are mandatory.
