Build an Online Store

I am looking for a landing page on my Website, that will give a customer the ability to process a HP Renewal order, this is either Via Credit Card or via Purchase Order, the web pages will need to be as laid out below, i have also attached an XLS of the layout i would like, and also a PDF with the order i would like the Pages to load to the client. We will not be holding any End User Card details, so will be Using merchant banking system. Please also ensure this is built in MySQL and PHP, to enable upload to my Hosting guys, i also need to have the ability to make changes and additions to the Pages myself. SCS Orders Website Spec What the website should be able to do: The ability to upload a CSV file to a MySQL database with unique information and this will be in the format of Company, Contact name, Serial Number, Product Number, expiry Date, Unique ID (this is what will be used to search on) and then a number of service levels with pricing for this Contact name/Company. For example: Company: Test Company Contact: Test Contact Serial Number: MXQ12345AB Product Number: AB123C Product Description: HP DL380 G9 Expiry Date: 21/02/2015 Unique ID: This should be generated when the records are uploaded Service Level 1: Foundation Care NBD Service Level 1 Price: £100 Service Level 2: Foundation Care 24x7 Service Level 2 Price: £150 Service Level 3: Foundation Care 6hr CTR Service Level 3 Price: £220 This would be one record and all this information would be generated by us apart from the Unique ID, which should be generated by the system There should be an ‘Admin Page’ to allow upload of the CSV File and download the CSV file with the Unique ID in! This should be Pass worded! There should be an end user form in which the end user would go to (for example: [login to view URL]) and type in the Unique ID and choose the expiry date from a ‘calendar’ and then click submit. This would then take them to a form with all the details displayed (that was in the database) and a radio button next to each service level. On this ‘form’ there should also be an option to pay for their order with by Credit Card or Purchase Order. There should also be an email address box for them to add their email address if they want email confirmation of the order(required for certificate), as well as a telephone number box. These should be added to the database to this record. They should not be allowed to continue unless either the Credit Card or Purchase Order has been selected along with a service level. If they select Purchase Order and a service level and wish to continue they will need to type in their name in a separate box as confirmation (or digital signature) that they wish to order, as well as their PO number. At this point an email will be sent to the End User and the sales team. The email to the End User will let them know what they have ordered and the email to the sales team it would let them know the same as the end user but also show the serial number and Unique ID. If they choose Credit Card rather than Purchase Order, then this would take them to the third party credit card processor and once they have completed the payment process it will take them back to the website and it would add the transaction ID to the database, to say they have paid. But this would also trigger the same emails as before….one to the end user to confirm they have ordered and one to the sales team with all the details and that they have paid for it by credit card.
