Build a Form page that ddds animated text at the beginning of a preset video

Its a single webpage that will contain 3 editable fields and 1 button : 1) To, Person B 2)YOUR MESSAGE, I dedicate to you this video because this is how you changed my life. 3) FROM, Person A 4) Generate button to generate the video link to share for copy and paste. Requirements: 1) There must be a requirement of Facebook login for us to pull the Image of Person A and list of friends on fb to be chosen as Person B. (this works same as [login to view URL]) 2) There must be only 1 fixed present animation same as old friends from [login to view URL] 3) The above message is set by default however should be editable. 4) The Animation will last No more than 10 seconds before the video from youtube plays. Video link will be given to you as its preset at the backend. 5) The video must be available after generating so please ensure its linked to a free online store system. video is already hosted on youtube so we will need to store the begining 10 secs animation only. 6) At the end of the video there must be a button so others can come to the same single page you created to dedicate to others.
