Adding form & editing contact form on Ultimo Magento template -- 6

We have a website almost completed on the Magento platform which uses the Ultimo theme from themeforest (the most popular Magento theme on the website).
We need to add a form to one page, which is pretty standard and will contain general information on the person submitting it, and will include a couple of drop down boxes. The form results are then to be emailed to an email address.
There is also an issue with the contact us page not displaying correctly, as the title appears at the top of the page and halfway down, so the one halfway down needs removing.
A solid understanding and experience working with Magento is essential, experience working with the Ultimo theme is a massive plus.
