2639 Clone a Simple Website

I am needing a clone of the following website: license [dot] shorturl [dot] com This is basically a joke website that looks like it comes from the department of national motor vehicles and says you can view anyone's license from the internet. Once a person visits they put in there name, state, and zip. It then goes to a site that says searching....and finds one match drivers license. Once a person clicks on the match it has a final page with a funny or fake driver's license with a monkey for a picture but there name they inserted in the drop down boxes now show up on the license. You'll have to visit this site and try it out for yourself before you bid. I need it to be identical in structure BUT I will be changing the graphics and name on the header. Not sure what coding language is used for this so again you will have to check out for yourself. ===================== ===================== Also there may be an opportunity to re-design the website as well to reflect a real gov site look but different that the original site.
