DEVELOP AN ONLINE FORM CALCULATOR APPLICATION/WIDGET WITH CONTACT FORM/LEAD GENERATION INTEGRATION

STAGE ONE: A WORKING ONLINE FORM CALCULATOR Online Calculator should utilize the following form fields: Industry (drop down menu/select) Gross Monthly Sales (Text input – numeric) Time in Business (Text input – numeric/date) Credit Score (Text input – numeric) Developing new code or using/modifying existing form functions, the results would select the appropriate response and populate fields accordingly. (See attached images for full layout.) STAGE TWO: CONTACT FORM INTEGRATION All values from user input and calculated responses should import into the contact database alongside user data collected through the online application and contact forms. Currently, we use Contact Form 7, but can be flexible as long as: User information is collected into a database, including hidden values such as timestamps and IP address. Reply/Confirmation emails are automatically generated utilizing custom HTML styles and attachments. Users are able to upload files as well as having those files linked to user information stored in the database (or similarly, the script should implement an auto-renaming process that affixes identifying information to uploaded filenames for easy retrieval and storage). OPTIONAL, YET HIGHLY DESIRED: All form fields collected by the database will prepopulate corresponding fields on the PDF version of the application form and is attached to the auto-reply email generated by the system. TO APPLY/REQUIREMENTS: Provide a copy of or a link to an electronic version of your portfolio or some samples of your work Must be familiar with WordPress and WordPress themes and child themes Have a general understanding of web coding languages such as PHP/HTML 5, JavaScript, etc. Be familiar with CSS style sheets and custom style sheets Produces great results expeditiously BONUS: Have a familiarity with the Genesis theme and framework, if necessary.
