FB app for booking my photo booth service

I need a FB app and/or application that I can use to book my clients and it will be similar to this $9 html/php/mysql script. [login to view URL] Yes, most of it is laid out already, I don't need all the time slots, I just need to know if they will book on a particular day. I have a photo booth rental service. Basically I have a set of questions (inquiry page like form) much like the sample above except with my set of questions and most importantly asks when the date of the rental is, the time the photo booth rental should start and duration of the photo booth rental, example, June 29, 2014, 12pm for 3 hours rental. I need to be able to log this information via admin side once client fills in the inquiry portion. While I save information for each booking, I should be able to see list of requirements to do and if I have done it put a date that it was done. For instance, I book a date as listed above, then I need to do following set of events for making sure I got everything done for each client booking. This can be as a checkbox form and then grey out once I have successfully check it off. Series of events for each event (which resides in the admin side), has nothing to do with client. Notice this is just a list of what to do. 1. Get a signed contract via scanned image/pic or in person 2. Get a deposit via paypal or in person. 3. Get all information for event criteria (4x6 or strip, colors used for design) 4. Create design template for event. 5. Get insurance for event. 6. Prepare balance payment for event. For each event, I should know when I finished each of the listed points 1-6 and see progress of what I need to do. As I book events, my app/php script should tell me if the date is available. I can book up to two events a day, but I need at least 3 hour window apart before or after to successfully book another event, otherwise I cannot book on that date. Example, iff I do an event July 29, 2014, and start 12pm and end 4pm, my next event if someone else book must start at 7pm or any event prior to 12pm must end by 9am. As I get booked clients should be able to see what dates are available by color coding available dates, or darkening/greying out unavailable dates.
