Need survey and tracking program completed (has great ongoing work potential)

I need a survey and tracking script that will allow user to create a survey, test the landing page, generate a custom sales page. I have a program started, I just need someone to finish it off to my specs. I know exactly what I have, and what I want. You need to have thorough understanding of: * php * js scripting * html * smarty * site graphic design and, you need to know how to integrate aMember, PayPal, and Authorize.net. 1. We are using JS scripting so that we can create and manage many surveys, w/ specific response showing just for those surveys. For example, if I had 25 different sites [domains] like [login to view URL] - I want to be able to manage each survey & response page from one place easy. I want the pages to be HTML page, I do not want people to have to try and use php pages. This way, every 'Project' that is created, create it's own JS script, and the person only has to paste the one piece of script: [login to view URL] and then just paste the 'tokens' like these: drawResponse(1); to pull in the responses. This *must* be very easy for someone with limited HTML skills to use. 7. I do not like the way the current app looks. I want a Step-by-Step process.
