32640 File Upload and manipulation i

I need a PHP or ASP script that will do the following. The script MUST work on an IIS system on a NT 4.0 server The HTML form I have will supply, the file path for the original file, this will be on a floppy disk, a Date param and a TermID param The script must upload the file to the server. Once uploaded the script must write a new line to the top of the file. This line will consist of the Date and Term ID param submitted from the HTML form and some static text. This is an example of the header line DiskDate:$DATE $DATE file for Terminal ID $TermID $TermID After writing the header to the file the script must MOVE the file to a new location. The location will either be a drive ( z:\path\to\move ) or a UNC path \\server\path\ I will own all rights to the completed code.
