Edit and Enhance an Adobe PDF Questionnaire Form to calculate a score and be digitally signed by recipient

I currently have an adobe PDF which is a form that calculates scores from a questionnaire I have tranto privde an end score to determine a risk profile. I have edited the static paper based form into a basic editable PDF form and now wish to enhance this further such that I am able to send it to client recipients via the Adobe digital sign process. The form needs to be able to: 1 - Calculate the score completed by the client recipient (Which has been already done in the standard adobe format) 2- Understand what risk profile the score relates to and output the profile result 3-If the client agrees to the risk profile by selecting Yes on the radio button then the form would move to the digital sign section. If the client chooses No then the reason needs to be entered by the client in the text box and selected risk profile is to be selected via a drop down box. The project requires for a capable adobe PDF script writer to help complete this form. IT is also expected that the radio buttons and font all are cleaned up to match the existing format of the file.
