We have various apps in Android market , we have a similar app where in users can request for an appointment , fill out date , time and any comments, we then sends out an email to office for actual booking. This application will be similar. Attaching photographs will be a new functionality but we should be able to achieve this. Normally an android developer account is required to launch any app , we can launch this one via our own account ( you will own the code). Please note look and feel will be very basic, we can add your existing logo to brand it and use similar colors.
