Create an html code for a link

I want to create a link (opens in a new tab) based on a drop down choice and an open text field. The link will always be (actual link provided to the winning bid) [login to view URL] the drop down will add: ?s= and the choice from select dropdowns The blank field will allow the user to imput their name and will add: &N= and then the name The end result when . you hit the "send" button will be [login to view URL] it will open in a new tab. Need this done in a couple of hours.
