Add PHP query, order, admins and poll form to sample website

i need a few pages of website (Use CSS for appropriate presentation of the pages) i need a query form, a order form, a poll form, a database and administrator page added to it. and in a way that it can be used in other websites, In php and javascript 1 = Query form: a form linked to a database and retrieve product information based on the criteria selected by the user.(product are computer accessories) 2 = Order form: a form linked to the database and save the data for each order received. 3 = Poll form: Data collected from the poll form inserted into a database table. 4 = Create an administration page to view (View All Products and View Poll Information) Appropriate form validation should be implemented for each form Other requirements: Client-side scripts to implement validation of form fields in forms where appropriate;Server-side scripts to implement the database functionality, Well developed and structured script, and appropriate and consistent comments throughout the code. The comments should be detailed Must have validation through W3school/validater
