Modify existing PHP page

Modify an existing PHP page. The page is behind an account login at www.linkwav.com. The target page is [login to view URL] Test account credentials to see the pare are JimDoetest / user pass / y78sdk41nd! Changes are needed to [login to view URL] 1. Change tab title to read only "Billing" not Account billing 2. Account information such as email, first name, last name, country, state and city used on the Profile page ([login to view URL]) should (optionally with a check box) populate any of the same fields in the Billing tab. Add check box to heading: Primary Contact Information (check box) Same as Profile Info 3. The Credit Card # should be on one line, not two. The fields could be made smaller in length so all four fields can fit on one line. Digits for the last four numbers of the card number to remain displayed as plain text. 4. When entering a credit card number, the cursor should automatically go to next field after entering first 4 of CC #, then next 4, etc. 5. Make two fields for credit card Expiration date. First field for expiration month with drop down menu to read "01 - January", "02 - February", "03 - March" etc. Second field for expiration year should 4 digits. 6. Add a dropdown list to select Visa, MC or Discover with heading "Card Type" to the left of the dropdown. Place this new field as first selection under the "Credit Card Information * required" title block. 7. Limit CIV code to 3 digits The deliverable is a modified PHP page(s) and any additional files as needed with the above described changes that we are able to upload to our server. After uploading to our server, the above described changes are in effect. In the case of adding a field to the Expiration Date, we will handle the mod to our database. This is a small project and should be done in less than 2 days. Bids over 2 days will not be considered. Thank you.
