DNN XML MSSQL form

This is a DNN project. I'm looking for a form to put on multiple child portals. This form would collect several items of information, ie: Name, Address, D.O.B., etc. Perhas as many as 20 data points

The application would then do a number things:

1. Create an XML query to an off-site database. This query will return XML approximately 10 more datapoints.

2. Add the XML datapoints to the record.

3. Generate an email to 1 or multiple recipients with all datapoints.

This application must be customizable for each child portal by the ADMIN level so that the ADMIN may:

1. Enter ACCOUNT field for each child portal. ACCOUNT would be in the XML query.

2. Enter PASSWORD FIELD for each child portal. PASSWORD would be in the XML query.

REGISTERED USER level should be able to see the data in their database AND ONLY thier database.

We have full XML specification available when we choose provider.
