I want the form page to be very, very user friendly. I also/want PDFs as an option, of my current contracts that the customer will be able to input information directly and forward the entire contract to my main email address in addition to an archive email address Once the customer navigates to the RSVP page, There needs to be options for the type of service that customer would like: Sedan service, limousine service, taxi service, van service, bus service. I want the form page to be transmitted to my email address with the appropriate title for the service that is desired. I want their title to display once they input their first and last name. Example: Title (dropdown menu; Mr. Mrs. Ms. Miss, Dr., Prof, etc) First name: 111 Last name: 444 Once they input & , the form should show a prompt of the time of day (if its afternoon) EXAMPLE: * Good afternoon, Mr.444. How can we be of service to you? * And then offer the customer the choice of: Limousine service, Sedan service, Airport service, Van service or other. * Then ask: "Would like to book this today?" or "Would you like a quote?" Depending on which option they choose will determine how the form page will be set up. If the customer selects "book this today" prompt for the following information: * Email address (*required) * Email address confirmation (*required) * Input title and name from initial inquiry, * Company (not required) * Address (*required) * City & zip code (*required) * Primary contact# (*required) * Secondary contact# (*required) * Fax # (not required) NEW PAGE HOWEVER YOU CHOOSE TO SETUP THE FOLLOWING IS AT YOUR DISCRETION. AS LONG AS YOU ASK THE FOLLOWING QUESTIONS: * Again start out with: (example) Mr. 444 we need to know: * Prompt for pickup date (using popup calendar that automates date input) (*required) * Prompt for pickup time (using popup clock that automates time input, in 30 minute intervals) (*required) * Prompt for pickup address (*required) * Prompt for # of people (*required) * Prompt for destination, options: airport, Amtrak, greyhound, airstrip, other if airport, ask which one (FAT, VIS, Chandler, SJC, SFO, OAK, SMF, LAX, Other airport) if other airport, ask for name of destination then prompt for the address or closest intersection then prompt for city or zip code if Amtrak, list local station (ask me for list of local stations) if greyhound, list local stations (ask me for list of local stations) if other, ask for name of destination then prompt for the address or closest intersection then prompt for city or zip code NEW PAGE Prompt: would you like to schedule a car for your return? If yes, then prompt for: * date of return * time of return * Which airline? * What is the flight# * What is your last "lay over" city? * What is your cell number? (return trips require a cell phone number) If no is selected, then continue to next section NEW PAGE * Again start out with: (example) Mr. 444 we need to know: * How would you like to pay for this service? Visa, MasterCard, Cash. * If visa or MasterCard prompt for: card# expiration CCV# prompt: THIS CARD WILL NEED TO BE IMPRINTED BY YOUR DRIVER. PLEASE HAVE the associated card AVAILABLE AT YOUR TIME OF PICKUP. * If cash, prompt: you will be provided a receipt. Then recap all input information. Finally ask: How would you like us to confirm with you? Email Phone call Fax (if selected, prompt for fax# and to whom to address the fax [SUBMIT] If the customer selects "Would like a quote:" repeat same as "book this today" EXCEPT WITHOUT payment section
