Convert a Template to a Website

Hi. I own the website [login to view URL] which is created in Wordpress theme Twenty Thirteen. I need someone to change the height of the header image to around 450px and move the menu to the top of the header toward the right hand side. Then I need an orange bar (hex ff6600) to run to across the bottom of the header image which includes the words 'GET FREE ADVICE FOR YOUR AUTHENTIC LIFE" in white text and a box for first name, a box for email and an orange button with the words "Join the tribe" (I will provide the button image for this) in it as the subscribe button linked up to my mailchimp account.
