Create mobile app using phonegap

I need a mobile app to be developed using phonegap and jquery mobile. Aim: 1. The purpose of the app is to store the date of birth of family and friend. 2. It should notify the smartphone owner using the app at regular interval using "notification" and subsequently open the application on clicking the notification 3. The exact time and interval between two notification must be editable using settings inside the app. 4. The app must also allow quick calling or emailing to the family or friend from inside the app. The app can obtain these information from the phones address book. Details: The app will have 3 pages. The layout of the 3 pages is shown in the enclosed document. The coloring scheme can be chosen from the jquery mobile theme roller available at [login to view URL] There are few instances where I would like to choose a color. Such choices are shown in the sketches. The operations need to be executed using phonegap. Deliverables: 1. The complete app along with all the required features. 2. Sufficient documentation 3. Ensure that the app is ready for creation of android and iphone apps that will work on the actual device 4. Source code
