Create Android Smart-Phone, iPhone, iPad App

I want to create an android app that will allow smart phone users to download prescription drug discount card to their phone by either going to a website or using a QR code. The app needs to work across popular platforms like iphone, ipad, etc. Here is an example (which is not obviously my company, but we offer nearly the exact same thing).

Since we'll have partners, I'll need to be able to modify the app to put their group code in (which is one of the variables of the card). If you need more to quote the job, please let me know. Thank you.
