calculation in wordpress, with result on a new wordpress page

Hi, i need a calculation module for wordpress. Based on some selection in dropdowns and one type-in value, you calculate and show the result on a new page. It should be easy for me to change the values, as the formula may change in some time. Layout should be uniform ([login to view URL]) field 1: dropdown with for each label a value field 2: dropdown with for each label a value field 3: number field, people can typ something here field 4: dropdown with for each label a value (conditional logic: only visible if field 2 has a specific value) field 5: dropdown with for each label a value (conditional logic: hide if field 2 has a specific value) hidden field 6: the value 4,5 (comma as decimal sign) hidden field 7: calculation: ((field 3 * field 2) + field 6)/250 hidden field 8: (field 7)^6 hidden field 9: field 8 * 4500 + field 4 + field 5 SUBMIT-button On a new page: field 10: field 9 * field 1. If the result < 40, than you show 40; if the result is higher than 10000, you show 10000
