One of my clients has lots of people going to their website and using a physician locator tool to find a doctor near them who perform a specific procedure. The telephone numbers listed within the locator tool are the doctor's own phone number, which means that there is no way of knowing how many people who go to the website actually call the doctor after using the locator. What I want to do is put a 1-800# with a unique extension for each physician in the locator tool. Here is what the phone switch would need to do; 1) when the people at the website dial the 1-800 and extension, the system reads the extension tone and uses that as a key to find the doctors direct line, then automatically forward the call to that number.. It can go out on a VoIP line. 2) the system needs to count and log the number of rings before the doctor's direct line is answered 3) the system needs to log time of day, extension #, number of rings before answering, and length of call to a SQL dbase 4) system needs to have a web-based admin tool that allows admin to create, edit, delete the lookup table that associates extension with direct line to be forwarded to. 5) system needs to allow for multiple "forward to" numbers, differentiated by time of day and day of week. For example, from 6:00 AM PST to 11:00 AM PST Monday thru Friday, forward to 415-555-1212, else forward to 415-555-1234. This will allow calls to go to the doctor's office when we know the nurse is available, to a call center at all other times. This needs to be configurable through a web interface 6) System needs to simple web-based pivot-tale reporting tool that sits on top of dbase created in #3 above, allows filtering by extension number, does simple summing of number of calls, total duration, etc. 7) I will supply all graphics for web admin interface, programmer to provide HTML and interface with datbase 8)Provider to help with install on Virtual Asterisk server
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).

I have created a fleshed out requirements document for this project, complete with wireframes, error messaging, and programming note. If you have looked at this project before and passed by because it was too loosely defined in scope you might want to look at it again and submit bid.
