Customer image upload and edit order form

Hello, I am desperately seeking to get designed a image upload form integrated onto current website, I currently use Dreamweaver to edit my website but I am unable to devise an upload form. We are a personised digital printers and would like a form to be setup so a customer can either select an image from our gallery of images or choose to upload there own for which a preview of the image is displayed. they then have the option to choose the size they require and any further adjustments they want done and to add there details. Then they are forwarded to a payment page (paypal) and directed back ot a thankyou page once completed. This would then be emailed to us. We are seeking a top quality finish like the form shown here [login to view URL] We would be happy to pay good money to match the finish on the above address. This would have to be integrated into our website for which I would supply you with the files. We are working on our new website now so the sooner we can get this form designed and integrated into our new website the better as we need to get this live. look forward to you replies.
