Appointment and scheduling software

I am running a clinic and I need my patients to be able to book online appointments, Basically the software will have 4 components: - Dashboard for administrator to modify the main settings (clinic time slots, schedule, days off, etc.) + users and user privilages. - Patient interface for the patient to book her appointment, She has to enter name, email and phone number and book an appointment in a very simple user friendly way (tick a date then tick an available time slot) for recurring patients she may enter phone number or email to book a new appointment or to cancel and rebook her appointment. Needless to say appointment, phone number, name, and email are all confidential data only seen by the docotr. - Doctor screen: - I may add appointments, block certain slots if I have an operation or meeting so they will not be shown available to users. - Today appointments will be shown, I would tick "visit" button or box to turn the appointment to an actual visits when the patient arrives or add a visit if a patients (who has no booking just walks in) "visits" are in actual real-time (patients who booked online and did actually come and patients who did not book online and come for consultation) I would be able to add "Notes about the visit" and the "Services done" and "Payment". (one patient may have 2 or 3 services -one to many relation) I would also add a noshow (patients who booked online but did not come). - I should be able to navigate to another screen showing previous visits, notes and payments of a specific patients. I do not want the software to accept payments online. -Clinic Secertary Screen (protected by usrname and passwords): Where the clinic secertary has privilages to can view, modify and revise appointments by day or by month...etc and tick patients arrived in real time . - Email reminders sent automatically 24 hours before the scheduled appointment - Reports generated from the database, mainly (Number of visits per day, month..etc) (Income daily, monthly..)
