Take a series of interview questions and create web pages with radio button responses that feed into a calculation to return an ROI number.?
Entry page will collect email name and email on a web-to-leads php form (supplied, but will need to be modified), and the exit page will email pdf files (supplied) to the person who filled it in.
Each page will have an explanation of the factor, a question, and response fields. ? Some will be numerical fill in the blanks, other will be multiple choice radio buttons. ? At the end, the values specified in the interview will be input to the ROI calculation (supplied). ? The finished button will take them to the exit page.

## Deliverables
Web pages will use the [url removed, login to view] style sheet and will also use the dreamweaver templates used on the rest of the site. ? (will be supplied).
