Modifications to an existing website for an insurance carrier that would provide agents access to bulletins and forms and other company materials. Some links wouldalso be used to products for rating use, but these are already built. There are some forms that we want to make more interact for use in sending data to the home office electronically.

## Deliverables
First phase of the project is to get about 1,000 forms already created into an organized retrieval pages so that agents can access company forms and download or print them. Due to the large number, I would like sort capabilites on the fields describing the forms. These fileds consist of a title, a description, a version date, an expiration date.
Additionally, several of the forms will need to be made interactive so that data can be entered into them, and the user would have a submit button that would send the form electronically to a location such as an email box that is linked to the form use. Also, I want to capture the users login name so that the return email can be added to the sent form in case there is additional correspondence required.
I would also like to have a submit button, a print button, and a save (download) button, and one that would do all three in one.
