convert php form and webpage to javascript

This is a pretty easy project we are capable of doing but don't have time because of time constraints in getting the client converted and transferred in time to meet their deadline. What we have is a form with various validations and subsequent html page that is written in php and html. We need the substantive information on the page (ie, the form info) converted over to javascript. FYI the javascript is not to be written on the page but called from a .js file on enter to the page. No page design is involved as we already have the page templates constructed. Once we are in negotiation with you as the possible winning bidder I'll provide the exact details regarding the form and how it is laid out including the existing code but that is a reasonably simple process as this is a reasonably simply project for a js professional. Here's the fine print. If I accept your bid you understand and agree that payment is only upon our receipt and testing of code that works 100% as the current one does. Failure is not an option for payment since this is a simple task for a professional so if you are not fully qualified please do not bid. Otherwise if you do bid and provide acceptable work we'll put you on the list for future tasks and we keep very busy so good chance we'll use you again.
