Custom Product Request/Contact Web Form

Looking for someone to create a custom webform for my website where clients can input their company info and product info and submit it....all requests will get emailed to my company. Need to be able to deliver the code in a way that I can just paste and copy into my website. Also, help me place it on the website if I have trouble embedding it there as I am not a web developer. I will supply a mockup in excel as to how the form should look, including formatting of fields, placement, arrangement, behavior, field hints, hover text, etc.
