build a form

I have a wordpress site its already completed I am using a contact form that uses a 3rd party autoresponder to send a link to the person who submits the form to download a ebook I want to replace this contact form using a local autoresponder and forward them internally to download the ebook I have installed contactform7 and flamingo to handle this I need you to build the form and replace my current form upon form submission the contact info will be saved (flamingo does this automatically?) The the user will be forwarded to the new page to download the ebook You will need to Add the page for downloading the ebook The form most appear the same as it is now meaning it has to look the same as it is now.
