Create an online registration database in LAMP

Must be done by next week Context and objectives We are doing market research and contacting several potential customers (all students) to ask them for a future service we will launch online in a few weeks. We would like to redirect those who will be interested by the service to an online registration form, so that we will be able to contact them afterwards once the service is launched. Therefore we need to create a very small website with 2 pages, to which people contacted will provide authentication information (like on a normal web-based service), receive a confirmation email, and record their contact information into a mysql database. The website will registe Scope of the work We will provide a debian server with SSH. The work will include : 1. Configuration of all required Apache / MySql / PHP services for the work to be done 2. Basic securing of the website; 3. Create & deploy necessary PHP & MySQL codes for the required registration service. Content of the work Technical constraints  The front-end will have to use the twitter bootstrap framework  LAMP is required for the rest of the site. Registration process to be implemented 1. Connection to the main page 2. (Possibility to log in to reconnect to the following process 5+ if interrupted) 3. Ask to enter email, password, password confirmation, and captcha; (mandatory) to create a profile; 4. Send confirmation email with a confirmation link and thanking for registration + link to access the registration forms (Steps 5+) 5. after step 2, access directly to the second page to ask for the following information (even without confirming email) : a. Last name, first name, (mandatory) b. Telephone number c. Current address d. Validate & save 6. Third page will ask for the following information a. Are you in (selection list) (mandatory) : i. Engineering school ii. Business school iii. Law school iv. Accounting school v. Administration school vi. Other (please indicate what) b. Which school are you in (mandatory) i. If the school is not already registered (not mandatory) 1. please enter the school address 2. Can you indicate us the contact information of the student office ? a. Name, email, phone number c. Which year are you in (mandatory) d. What would you say is your personal specialty ? (non mandatory) e. Validate & Save 7. Possibility to go back for edition for correction 8. Record information into a corresponding database a. The date & time of registration will have to be recorded as well. Consultation process to be implemented 1. Connection to the main page 2. Connect with administrator profile 3. Create & download a CSV file on request gathering data base information, one line per person, with all information in rows. 4. Log out. Design, etc… The design would have to be similar of the website Linkedin, comply to the following requirements : 1. Grey-black banner on top 2. Grey background 3. White-cream interaction windows. 4. Leave space for us to add graphics and text around the registration window You can use twitter bootstrap for forms & button.
