design few web pages ( html forms/php/css/graphic )

You are not required to develop database part. I need a good graphic + css designer /php coder who makes the wep-pages attractive. Need a simpler version of forms on [login to view URL] The following forms are required : search coordinates by postal code state/province postal code listings distance calculator between 2 postal codes on the header that should be on each page there must be links to the other pages . On submit each form should redirect to result pages hidden variables: form_name and values selected or typed in the links and on the forms do not put absolute paths - put relative path instead using unix style delimiter - forward slash - if needed. ( for simplicity you can consider all search and result pages located in same folder ) Result pages: do not code how to actually display results but design nice layout in plain HTML+css for showing lookup results for each type of search. with some dummy data. There should be also links to prev/next N records if searches return more then 1 page of results. Designers and Coders having good portfolio will be preferred. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law.
