Data collection application(repost)

I am looking for an expert ASP.NET programmer to program a data collection application. The application will basically have the following components: 1) A set number of fields (about 40) from which users can select which ones they want to include on an online form. Once they select the fields that they want, an HTML form will be generated for them using those fields. 2) The users should be able to organize the form's fields so that they are displayed in the order they want them to be. 3) The form will then be embedded into an existing website using Javascript. 4) When visitors fill in the form, the data will be submitted to an SQL Server database. 5) The result records will then be displayed to the form administrator. The list will be organized by either date or name of respondent. 6) All data should be exportable to CSV format, so that admins can import the data into Excel or Word. 7) There should be a button that enables admins to auto-update the database, by sending out each contact's record to them via email to verify that it is up-to-date. 8) Each contact should be able to login via the link in the email and update his or her contact information, inside the form it was originally entered. The application should be built to allow for many different accounts, each with its own custom forms and admins. The entire application should be programmed using VB.NET and will run on SQL Server 2000. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform Windows Server 2000
