We need to redo a file upload page so our clients can uploadarge files (up to 200 MB). The web page should have 5 upload fields for files and name, address, phone #, paper type, print size as well as filename fields. We need to attach the customer's last name to the file names he is uploading so the customer does not have to bother typing the file names. Once the files are uploaded to the server, e should be able to discern whom the file belongs to. Also, we would like toreceive an e-mail notification of the form submitted. Last but not least, the customer should see a progress bar indicating upload progress. The uload page will have the same design as the rest of the site (the site is html coded), The confirmation page should inform the user the file(s) have been uploaded and have a link to go back to the upoad page if necessary. We would like this to be done in .asp. Our site is hosted on a Windows server.
## Deliverables
1) Complete and fully-functional working asp page as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
