Code a form on my website that confirms permission to send marketing emails.

I wish to re-confirm recipients of an email newsletter list. I would like to send an email to the list. By clicking on a "I Consent" link, the recipient confirms that he/she wishes to continue receiving the emails. The page should capture the recipient's email address, date and time of the click and the IP address. These should all be saved to a CSV file. There is no need to style the page. I will add styling elements. My server is using Apache, MySQL and PHP. The body of the email reads as follows: On July 1, 2014, Canada’s new anti-spam law (CASL) comes into effect.
