Boonex Signup Form

We have a Boonex community. We need another "Join page / Signup page" created that is simple and only asks approx 5-7 questions. We have a Join Form right now that is very long. We want to keep that in place and give people the option to Join using the "Advanced/Long-Form" if they want. The new additional "Join page / Signup page" will have questions very similar to the Facebook signup page (first name, last name, email address, password, confirm password, company name). We need this created quickly because of an immediate need.

We need to be able to copy this page and re-brand it for different companies.

If the current Join Form is [url removed, login to view], we want the new simple form to be something like, [url removed, login to view]
